Retirees must choose or some could lose coverage
If you are insured through Arizona State Retirement System…
If you do not wish to make any changes under the ASRS medical and dental plans, you do not need to complete a new enrollment form.
However, most premiums will increase and there are some important changes to the non-Medicare plans, so retirees are encouraged to attend an enrollment meeting for additional information.

Retirees will receive a new card if they change to a different plan, or remain in the Group Medicare Advantage and non-Medicare Choice plans. Retirees who continue with the Senior Supplement and Navigate plans will not receive a new card.
If you are insured through Arizona Department of Administration…
You must take action!
Open enrollment ends November 17 at 5 p.m. Arizona time and selected coverage will be effective January 1, 2018.
Retirees covered through the Arizona Department of Administration must complete a new enrollment form in order to continue ADOA coverage. Those who do not re-enroll will lose coverage permanently January 1, 2018.

Enrollment meetings & consultations
ASRS will hold separate non-Medicare and Medicare open enrollment meetings. Please check your PSPRS guidebook for the meeting schedule and locations or click here for the non-Medicare plan meetings and click here for Medicare plan meetings.
You can also call PSPRS at (602) 255-5575 to make an appointment with Retired Member Services staff, or for ASRS-covered retirees only, to schedule a consultation with a United Healthcare representative.

Open enrollment FAQ’s
- If you are not sure if you are enrolled in the state plans or what plans you may have, call our office at (602) 255-5575 to speak with Retired Members staff.
- If you are going to make a change, fill out the application and send it to the PSPRS office at 3010 E. Camelback Rd. Suite 200, Phoenix, AZ, 85016.
- If you want to end your state medical or dental coverage, sign and send a letter to request termination to PSPRS.
What if I did not receive a packet?
- Contact our office at (602) 255-5575 to speak with Retired Members staff.
- Go to our website where you will be able to access the PSPRS guidebook, see when there are meetings and print an application if you are wanting to make changes.
- Plan on attending an open enrollment meeting hosted by a UnitedHealthcare representative.
- Call one of the United Healthcare representatives on site at Arizona State Retirement System (ASRS) to get more information or set up an appointment. You can speak with Amy Potter at (602) 240-5310 or Cindy Gonzales at (602) 240-2113. Make sure to tell the representative that you are a public safety retiree so that they don’t try to look you up on the ASRS system.
- Our office is open from 8 a.m. untill 5 p.m. Monday through Friday except on federal holidays. Please note we will not accept walk-ins after 4 p.m.
- If you call our office and ask to speak to a Retired Members staff member, please be patient. Due to heavy call volume during open enrollment, you may have to leave a voicemail. If so, please state your full name, phone number and the last four digits of your Social Security number. This will allow us to pull up your record and have it ready to go to help you when we call.

- Subsidy amounts
- Deduction amounts
- Contact information for United Healthcare or SunLife Dental
- Insurance claims or doctor information
- Change of address
- Verify your coverage plan
- Print out insurance information
- Yes, whether you are already on the plans or are thinking about taking one of the state plans, you should go to the meetings as there are significant changes within the plans to be aware of.
- Yes, they are officially now known as SunLife.
- No new cards will be sent out unless you enroll for the first time or change your plan.
