If you are either: (a) a member of the Plan who, on or before July 20, 2011, either (i) had your last day of employment under the Plan or (ii) filed an application for benefits under the Plan and have been found eligible for those benefits, whether the determination of eligibility was made before or after July 20, 2011; or |

(b) a spouse or surviving spouse of a member described in (a) above; or |
||||||||||||||||
(c) a child under the age of 23 of a member described in paragraph (a) above, or a disabled child of a member described in paragraph (a) above whose disability began before the age of 23 and who remains a dependent of the member, a surviving spouse, or a guardian.
